Any Nursing Assistant can be nominated using the online survey, to access the survey click the "Nominate an Assistant" link on the left side of this screen. To view and print off the questions that will be asked on the form, click here. Please remember to fill out the form in full with all required fields submitted. Also, the Administrator or Director of Nursing at the nominee’s facility must validate the nomination by completing a brief online validation form.  The nomination will not be processed until it has been validated.

Supervisors will receive an email after the nomination has been submitted and will have 14 days to complete the validation.

Nominees and the facility's management will be notified by letter and email when both the nomination and validation have been received and submitted to AANAC for judging.

The nomination period begins July 28, 2008 and closes January 31, 2009. Sorry, but any nominations received after February 1, 2009 will not be eligible.
